How to Request an Appointment
For your convenience a direct referral line is available from 8:00 a.m. – 4:30 p.m., Monday through Friday 800-444-8314 or 859-268-5666. Calling us is the best way to request an appointment and allows us to schedule the appointment at a date and time that best suits your schedule.
If you prefer to request an appointment for evaluation by mail, there is a download of an “Admission Application” for you to complete and mail to us.
This form is a request for an initial evaluation, not for “admission” to the hospital as an inpatient. If you chose to complete the form, you do not have to fill in space #9 for a sponsoring Shriner. Please send the completed form to:

All requests for care are reviewed by the Chief of Staff to assure that the child has a condition we specialize in.
- If your child’s condition is one that we specialize in, a packet of information will be mailed to you which includes the following:
- a letter noting the initial appointment date and time (it will include the name and telephone number of a contact person at the hospital for you to call if you have questions about your first visit)
- several pre-registration forms
- a checklist of things to bring on your first visit
- a map and directions
- listing of our affiliated Shrine Canters that provide transportation to the hospital
- If your child has a condition that is not within our scope of expertise, you will receive a telephone call from our patient services coordinator explaining that we do not specialize in the service your child needs. If we are unable to reach you by telephone, you will receive a letter.
Your First Appointment: What to Expect
Physician offices and support service (X-ray, lab, physical/occupational therapy, orthotics, prosthetics, social services, care coordination, etc.) are all located in the hospital.
After the registration process, your child will receive a thorough assessment and diagnostic testing as indicated by his/her condition. Then you will meet your attending physician, this is the doctor you will see each time you come to the hospital. Your doctor will talk with you about your child’s diagnosis and the treatment we believe is needed. There will be time for you to ask questions and get them answered. Also on the first visit, you will meet your care coordinator. This person is a nurse, with expertise in the condition your child has, who will assist you as long as your child is a patient here. Your care coordinator plays a key role in assuring that what we are doing for your child is coordinated with other health care providers your child may be seeing.
Outpatient Clinic
Our outpatient clinic is like a doctor’s office in the community – initial assessments and follow-up appointments all take place in the outpatient clinic. Diagnostic and therapeutic services the physician may order (i.e. radiology, laboratory, physical and occupational therapy, orthotics and prosthetics), are located in the hospital – a real convenience for busy families.
Each patient is assigned a care coordinator, a nurse who will assist the family as needed and assure that care rendered at Shriners Hospital is coordinated with services the child may be receiving from other health care providers in their home community.

Overnight Accommodations
Parents are encouraged to stay with their child during hospitalization. Sleeping accommodations are available in each room, or in the parent quarters. On-site sleeping space is limited, therefore, we ask that only one parent or caregiver stay overnight at the hospital.
If off-site accommodations are desired or needed, the best way to locate a hotel that suits your needs is to perform a web search (the hospital address is 1900 Richmond Road, Lexington, Kentucky 40502).
Lexington has a wide range of lodging available in the city. Some hotels offer discounted or hospital rates. If the hotel requires proof of appointment for the lower rate, we can furnish the appropriate documentation.
