If your child has a condition treated by Shriners Hospitals for Children — Shreveport, please contact our intake/referral coordinator to schedule an appointment. Your intake/referral coordinator will partner with you to gather the necessary information to evaluate whether your child can benefit from care at our hospital.
Please bring with you:
- Insurance card
- Primary care physician’s or clinic’s name and address
- Referring physician’s name and address
- Medication and dosages list (including over-the-counter and herbal) your child is currently taking
- Test results, X-rays or any other records completed after your medical records were sent
- Any orthotic or prosthetic devices your child currently uses

We appreciate receiving your child’s medical records prior to his or her appointment. If you have medical record notes, biopsy slides, lab results, X-ray films, and CT or MRI reports, please mail or bring them with you to your appointment.

Outpatient clinic patients
Information about your clinic visit (outpatients)
- You and your child will be introduced to your care manager on your first visit.
- Your care manager will be your family’s contact throughout your child’s treatment and will answer any questions or concerns you may have.
- Our staff makes every effort to provide your child with the best possible care in a family friendly setting. Because we spend time discussing issues with you and getting to know you and your child, your appointment may take up to four hours. Your visit may take longer if further tests are needed during a busy clinic day.
- If your child should require diagnostic testing, consultation with a specialist and/or has equipment needs not provided by Shriners Hospitals for Children — Shreveport, outside resources will be utilized. The hospital will contact your insurance/state aid providers to assist in the payment of these expenses.
- Your child may be examined by physicians and other members of your child’s medical team. This may include nurse practitioners, physical therapists, occupational therapists, recreational therapists and/or child life specialists, pain management professionals, respiratory therapists, radiology technicians, orthotics and prosthetics technicians and anesthesia experts.
- Please bring a pair of athletic – zipper-free – shorts for your child to wear during examination and X-rays, if appropriate. For patients with long hair, please also bring a hair tie.
- During your visit, your physician may request that photographs be taken of your child to record his or her condition visually. This visual record will assist your physician in evaluating your child before, during and after treatment, and is often very helpful in documenting subtle changes that may occur as your child grows. For photographic clarity, your child may be photographed wearing minimal clothing. This is necessary so the doctors can see as much of the body as possible.
- If your child is ill or has been exposed to a contagious disease such as chicken pox or head lice, please notify our clinic scheduler at 318-226-3313 immediately to reschedule your appointment.
If you have questions about your child's scheduled surgery or the admission process, please call 318-226-3313.
Please notify us if, within three weeks of your surgery admission date, your child experiences symptoms of infectious illness including fever, cough, rash, vomiting, diarrhea or any suspected/known exposure to chicken pox, measles or any other contagious disease/condition.
Surgery pre-admission visit
You can expect a phone call from your care manager one to four weeks prior to your surgery admission date. If for some reason you find it necessary to cancel this admission, please let the outpatient department know as soon as possible. A pre-admission appointment will be scheduled before your admission date.
We will try our best to make your child’s stay with us as easy as possible. Please plan to arrive at the time designated on your admission notice. During this visit, you will meet with the admission nurse who will review your child’s health history. Although the health history review may seem repetitive, your answers help us give your child the best care possible.
We realize your presence during your child’s stay at the hospital is very reassuring, so we encourage one parent/guardian to stay at the bedside.
Additional rules and tips:
- Because our waiting space is limited, we ask that you limit the number of family and friends accompanying you to your child’s appointment.
- Use of tobacco products is not permitted in the hospital or on the hospital campus. Those who smoke (tobacco products and vapor products) or use chewing tobacco must leave the hospital grounds to do so.
- Only mylar, non-latex balloons are allowed in the hospital, in order to provide a latex-safe environment to our patients who have allergies.
- Our hospital does not have a gift shop. Please let your friends and family know that if they wish to send a gift of flowers, mylar balloons or other items to your child during his or her stay, that there are numerous local flower shops in the Shreveport area that can provide this service.
- No pets are allowed except for service animals.
- The hospital is not responsible for lost or stolen items.
- We kindly ask that you ensure your child’s vaccinations are up-to-date before visiting the hospital. Please note that the hospital does not provide immunizations to any of our patients or their family members.
