Our professional team will ensure you receive appropriate education regarding your child’s condition, treatment and ongoing care needs. Patients and families are encouraged to share their questions and concerns with their care teams.
In the two weeks leading up to your admission date or scheduled surgical date, if your child experiences a fever, cough, rash or redness of skin, night sweats, itching of body/scalp or lice, sore throat, vomiting or diarrhea, or exposure to chicken pox, measles or upper respiratory illness, please call our outpatient clinic at 773-385-5890, Monday through Friday between 7:30 a.m. and 4:30 p.m. A nurse will talk with you about these concerns.
A parent or legal guardian must accompany your child on the day of admission. If legal guardianship is held by someone other than the biological parent(s), the legal guardian must bring guardianship papers with them to the hospital. If custody is granted to one parent by the courts, a copy of this court order will also be needed for our records. The papers must be in English.
If you do not speak English, please bring an interpreter.
When you arrive at the hospital, please check in at the reception desk in the lobby. Do not go to a patient care unit.
