Patients and families are encouraged to share their questions and concerns with the care teams as outlined in the Patients’ Rights and Responsibilities brochure that will be given to you at your first visit.
Please notify us within three weeks of the surgery admission date if your child experiences symptoms of infectious illness, including fever, cough, rash, vomiting, diarrhea or any suspected/known exposure to chicken pox, measles or any other contagious disease/condition. Call our outpatient clinic at 213-388-3151, Monday through Friday between 8 a.m. and 4:30 p.m. A nurse will talk with you about these concerns.
A parent or legal guardian must accompany your child on the day of admission. If legal guardianship is held by someone other than the biological parent(s), please bring guardianship papers with you to the hospital. If custody is granted to one parent by the courts, a copy of this court order will also be needed for our records.
If the patient will be accompanied by someone other than a legal guardian, a notarized permission slip signed by the legal guardian must be presented at the time of appointment check-in.
