Health precautions

If within three weeks prior to date of surgery, your child experiences any of the following conditions, please call care management Monday through Friday between 8 a.m. and 5 p.m at 801-365-3777.

Your child’s safety is our number one priority. You can help us by being active, involved members of the health care team.

Patient responsibility

If someone other than the biological parent(s) holds legal guardianship, please bring guardianship papers with you to the hospital. If custody is granted to one parent by the courts, a copy of this court order will also be needed for our records.

When you arrive at the hospital, please check in at the reception desk in the lobby.
