It is our belief that effective communication and strong partnerships help to provide quality, seamless care to the children we serve. Please contact our professional relations coordinator if you have questions, need assistance or would like to schedule an office visit with one of our staff.

Refer a patient

Referring patients to Shriners Hospitals for Children — Erie is a simple process which only involves a phone call. Any child age 18 or under is eligible for care, although in some cases care may be extended to age 21. The child’s condition needs to be within the scope of services provided. All children are treated regardless of the families' ability to pay.

To refer a patient to Shriners Hospitals for Children — Erie, please contact a referral coordinator and you will be guided through the admissions process.

Please have the following information available:

  • Child’s full name
  • Date of birth
  • Legal guardian contact information
  • Diagnosis or chief complaint
  • Insurance provider, identification number and phone number
  • Primary care physician
  • Referring physician, if not primary provider

Please forward any applicable laboratory results, x-ray films or CD and/or other diagnostic studies.
