Patient referrals

Referring or transferring patients to Shriners Hospitals for Children — Honolulu is a simple process which involves a phone call or fax. Any child under the age of 18 is eligible for care as long as the child’s condition is within the scope of services provided. Please see the conditions we treat.

Refer a patient for orthopaedic care

Shriners Hospitals for Children — Honolulu has four full-time pediatric orthopaedic surgeons. Our specialists help children with orthopaedic issues from the neck down. To refer a patient, please contact our outpatient clinic and you will be guided through the referral and admissions process.

What the parent/patient should bring:

  • Parent/guardian photo identification and insurance card (if applicable)
  • Copies of X-rays if available on film or disc
  • Patient immunization record if available
  • Single parents who are divorced should bring their divorce decree
  • List of patient’s current medications and dosage

Please note that Shriners Hospitals for Children — Honolulu accepts all children with or without insurance and regardless of the families' ability to pay. No special sponsorship is needed.
