If your child has an upcoming appointment and you or your child are exhibiting a cough, fever, chills, shortness of breath or flu like symptoms, please contact your local Shriners Hospitals for Children location to reschedule the appointment.

Medical records

Physicians who refer patients to Shriners Hospitals for Children — Houston will receive a note from the patient’s first visit. If other notes are needed or you have any questions, please call health information management at 713-793-3939.

Shriners Hospitals for Children — Houston believes in the continuity of care between health care professionals. If you need medical records on a mutual patient for continuing care, please call health information management at 713-793-3939 or fax a business cover sheet with the details of your request to 713-793-3770 and we will be happy to process your request.

Patient family and other medical record requests

Families and others may request records through health information services by calling 713-793-3939. An Authorization for Disclosure of Health Information form must be completed for us to process these requests. We can also fax, email or mail a copy of the authorization form.
