If you are requesting copies of you or your child’s Shriners Children's Twin Cities medical record, please follow these instructions:

  1. Download and print this Authorization for Disclosure of Health Information form
  2. Fill out the form as completely as you can.
    • Be sure to include the patient information, #1, #2, #3 and signatures.
    • Please be as specific as you can about the information.
  3. Please email, mail or fax your authorization form to the address below.
    • There may be a charge for copies of your medical records. If there is, we will notify you before copies are made.
    • Requests for copies may take between 10–15 business days to process upon receipt of the authorization form.

Please be advised that records can be transferred between Shriners Children's Twin Cities facilities by calling the health information department at 612-596-6154.
