Medical record information

To request copies of your medical records, please contact health information services at 514-282-6981 between our office hours of 8 a.m.–4 p.m., Monday through Friday.

We will ask you to complete an authorization form to release the information form and mail it to:

Requests for records usually take 20 days to complete; however, depending on the size of the records, it may take as long as 30 days to complete.


At Shriners Hospitals for Children — Canada, we provide medical services without barriers. Our practitioners determine the plan of care for all families regardless of their ability to pay.

Insurance for patients from the U.S. or other countries

We accept many types of insurance and offer options for financial assistance to those who need help or are uninsured. Your care manager will connect you with our finance department if you have questions about insurance, costs and coverage.

For each inpatient hospitalization, the Canada Shriners Hospital is required to contact your insurance provider to notify them prior to the admission date. Our patient financial services staff handles the notification process.

Services performed outside of Shriners Hospitals for Children — Canada

Occasionally, a patient will need services performed that cannot be completed at the Canada Shriners Hospital. This includes, but is not limited to MRIs, CT scans, genetics counseling, testing and some surgeries. For patients coming from the U.S. or other countries, your insurance provider will most likely require an estimate of care costs from the Canada Shriners Hospital.

Please note that the Canada Shriners Hospital cannot assist you with payment on your prescriptions, select medical equipment and any rehabilitation therapies you receive from other providers that are not affiliated with us.

Frequently asked questions about medical records, insurance and billing

Do I have any out-of-pocket costs at the Canada Shriners Hospital?

The only costs will be your personal incidentals. Our social services staff work with the Shriners International fraternity to cover most of the lodging, transport and meal costs.

Does the Canada Shriners Hospital accept my insurance?

For patients from the U.S. or other countries, Shriners Hospitals for Children — Canada accepts most insurance, including Medicare. You may contact your insurance provider to see if we are considered an in-network or out-of-network provider.

What if I come from a province other than Quebec or the U.S.?

At Shriners Hospitals for Children — Canada, we see children from every province and a portion of the cost of care is billed back to your home province if you are not from Quebec. We have many ways to assist with difficult financial situations. Our social services staff will assist you in finding medical coverage or financial assistance. For U.S. families, we offer charity care and/or Shrine assistance for those who do not qualify for private or state-funded health benefits.

What happens if I do not have an insurance referral?

There is no need for an insurance referral at the Canada Shriners Hospital.
