Physicians who refer patients to Shriners for Children Medical Center — Pasadena will receive a note from the patient’s first visit. If other notes are needed or you have any questions, please call or fax our health information services at 626-389-9500/ fax 626-389-9501.

Shriners for Children Medical Center believes in the continuity of care between healthcare professionals. If you need medical records on a mutual patient for continuing care, please call medical records or health information services at 626-389-9500. We will be happy to process your request.

Family and other requests

Families and others may request records through health information services by calling  626-389-9500. An Authorization for Disclosure of Health Information form must be completed for us to process these requests.
