To request copies of your medical records, please contact Health Information Services at 626-389-9500.

You will be asked to complete an Authorization for Disclosure of Health Information form, which we can send to you via email or fax, or you may download an English or Spanish version. If you prefer, you may pick up a form in person by stopping by the health information services office during our regular business hours: 8 a.m. – 4:30 p.m., Monday through Friday.

Requests for copies take 10–15 business days to process upon receipt of the authorization form.

Insurance questions

Children up to age 18 are eligible for care if, in the opinion of our physicians, there is a reasonable possibility they can benefit from the specialized services available. Treatment is provided based on medical need, regardless of the families’ ability to pay.

We accept medical insurance, including Medi-Cal, managed care and private payer. However, insurance coverage is not a criterion for care. Any child under 18 with a medical condition that is within the medical center's scope of services is eligible.
