Insurance questions

At Shriners Hospitals for Children — Portland, there are no barriers to care. Patients receive the best course of care as determined by their doctors – regardless of the families' ability to pay. We accept new patients with or without insurance coverage. Any child under 18 with a medical condition within the hospital’s scope of services is eligible for treatment.

If your child is covered by private insurance or a state funding plan, such as Medicaid, the hospital will bill and accept reimbursement for services provided at our hospital. Please have your insurance information available when you call to make an appointment.

Our hospital also provides financial counseling to explore coverage options and help you navigate the cost of your child’s care. If the cost of your child’s care creates undue hardship on your family, please contact the financial counseling department at 503-221-3702.

To request a copy of you or your child’s medical records please download and complete the Authorization for Disclosure of Health Information form.

You may fax your completed form to 503-221-3713 or mail to:

Please note that a form of identification must be copied and sent along with your completed authorization. Failure to do so may result in the delay of processing your records. 

If you prefer, you may stop by and pick up an authorization form in the health information management office during our regular business hours, Monday – Friday, 7:30 a.m. – 4 p.m.

Requests for medical records may take up to 30 days to process upon receipt of the authorization form.
