At Shriners Hospitals for Children — Portland, we value relationships with our referring providers. It’s our belief that effective communication and strong partnerships help to provide quality, seamless care to the children we serve. Please contact one of our physician relations team members if you have questions, need assistance or would like assistance to schedule an office visit with one of our staff.

New patient referral process

There are two ways to refer a new patient:

  • Call our new patient referral department
  • Fax your patient's demographic page and relevant chart notes to our new patient referral fax line and we will call and schedule your patient.

After we receive the referral, our new patient intake coordinators will contact the family to set up their appointment. Once the appointment date is established, you will receive an appointment confirmation letter in the mail.
