As part of our mission to provide the highest quality care, we want to make it easy and convenient for you to have safe, secure and continuous online access to the most up-to-date health information.

Getting started

If you have not yet received a Patient Portal username and password, please complete Patient Portal enrollment at your next appointment.

Following your appointment, you will receive an email with login and registration information.

Mobile app

You may download the HealtheLife app from the Apple App Store or Google Play Store. The app is free and compatible with IOS and Android devices.

Request for Medical Record Release

You may request a copy of your medical record from the Patient Portal. Review Request for Meical Record Release via the Patient Portal (English) (Español) for instructions on how to submit a request.

Information available in the Patient Portal

The Patient Portal provides you with access to key health record items, including:

  • Vital signs (height, weight, BMI)
  • Known allergies
  • Appointments/calendar
  • Educational materials
  • The reason for your child’s visit(s)
  • Certain lab results
  • Radiology reports
  • Hospital visit summaries
  • Current and resolved health issues
