Telehealth Affiliate Requirements

Telehealth Affiliate Requirements

To be considered as an affiliate site for the Shriners Hospitals for Children Telehealth Care Network (TCN) expansion, the affiliate site must meet the minimum requirements identified below:

  1. Located in an area where Shriners Hospitals for Children (SHC) patients are located, or can be more conveniently accessed than an SHC location. The affiliate should be located outside of the SHC location local service area and identified by SHC location with assistance from headquarters. Affiliates present patients to the SHC location during the telehealth encounter, increase the number of children, families and pediatric patients served by SHC, increase referrals to SHC, and deliver an exceptional experience with emerging telehealth technology, processes and operations. (Note – there is a maximum of two affiliates for an SHC facility's first year of Telehealth)
    • SHC will place and manage a Telehealth cart at the affiliate site to perform the telehealth encounters
  2. The facility is staffed with physicians and certified medical professionals (e.g., nurses, nurse practitioners, etc.)
    • The affiliate shall have a registered nurse (RN), physical therapist or physician assistant/physician to present the patient to the SHC location/physician during the telehealth encounter
  3. The affiliate site is a health care facility – a hospital, ambulatory care center, outpatient facility, physician office, Federally Qualified Health Center (FQHC), rural health clinic, skilled nursing facility or rehab facility. Note: a Shriner center is NOT a desirable facility due to imaging needs
  4. The facility offers opportunity to reduce Shriner-funded transportation expense through the use of telehealth
  5. The facility can handle a sufficient volume of orthopaedic, burn and other procedures that support telehealth services and use cases
  6. Medical staff at the facility have knowledge of pediatrics and/or burn and orthopaedic needs, and can effectively present the patient to the SHC care team
  7. The facility will be ready to begin conducting telehealth encounters within 2019, and is aligned with the SHC location clinical go-live timeframe
  8. The facility will make space available for a multi-use room optimized for telehealth encounters. The space should be at least 12’ x 14’ (168 square feet), with access to necessary power, lighting, HVAC, and data and voice connectivity. When not in use, the telehealth cart shall be stored in a secured closet
  9. The facility is able to meet the patient/family telehealth care access needs and will provide expected workflow and an exceptional telehealth experience
  10. The facility has ample network bandwidth and technical infrastructure to support the telehealth communication needs
  11. Ideally, the facility is located in a state with a telehealth parity law and the state has regulations, licensing and reimbursement that is supportive of telehealth
  12. Ideally, the facility has previous telehealth experience, but is not required
  13. Ideally, the facility can offer imaging and lab services, or required imaging services are available in an adjacent space or in the immediate regional area
  14. The facility is interested and motivated to be an affiliate with SHC, and can quickly execute an agreement
  15. The facility has local SHC location executive support and champion for use as a telehealth affiliate site
  16. The facility does NOT pose a competitive threat and offers complementary clinical services
  17. Could lead to a meaningful partnership, in the form of referrals or other mutual care benefits
  18. Facility is willing to allow staff to receive telehealth and equipment training provided by SHC, as needed
  19. In the event of an emergency, the facility could either assist with the emergency or provide transportation to a care facility within close proximity
  20. The facility would NOT require burdensome licensing and credentialing requirements
  21. The facility can provide opportunity to serve more patients and access new markets
