Physicians who refer patients to Shriners Hospitals for Children — Shreveport will receive a dictation note from the patient’s first clinic visit. If other dictation notes are needed, please call the medical records department at 318-226-4268.

Shriners Hospitals for Children — Shreveport believes in the continuity of care between health care professionals. If you need medical records on a mutual patient for continuing care, please contact the medical records department at 318-226-4268 and we will be happy to process your request. Authorizations to release medical records may be faxed to 318-424-4561.
