Please note that Shriners Hospitals for Children does not administer COVID-19 testing or treatment at this time. If your child has an upcoming appointment and you or your child are exhibiting a cough, fever, chills, shortness of breath or flu like symptoms, please contact your local Shriners Hospitals for Children location to reschedule the appointment.

Medical records 

To request a copy of your child's medical records, please download the Authorization for Disclosure of Health Information form (Authorization for Disclosure of Health Information (ESP)) and fax the completed form to 413-735-1226, or mail to:

If you are a former patient requesting a copy of your records, please provide a copy of your driver’s license along with the form.

If you prefer, you can stop by the hospital and fill out the form in person anytime between the hours of 8 a.m. – 4:30 p.m., Monday – Friday.

Requests for copies of your medical record take 10 to 15 business days to process upon receipt of the authorization form. There may be a charge depending on the size of the request.

Insurance questions

At Shriners Hospitals for Children — Springfield, there are no barriers to care. Doctors determine the course of care and all care is provided regardless of the families’ ability to pay. If the child is covered by private insurance or a state funded plan, the hospital will bill and accept reimbursement for services provided at our hospital. However, insurance coverage is not a criterion for care. Any child under age 18 with a medical condition that is within the hospital’s scope of services is eligible.
